Hypothyroidism is a common endocrine disorder in which the body does not have enough thyroid hormone to meet its physiological needs. Because thyroid hormones influence energy production, temperature regulation, cardiovascular function, gastrointestinal activity, brain function, muscle activity, and many other processes, deficiency can affect almost every organ system.
The thyroid gland produces mainly thyroxine (T4) and smaller amounts of triiodothyronine (T3). Much of the circulating T3 is produced when T4 is converted to T3 in peripheral tissues. Thyroid hormone regulates the body's metabolic activity. When hormone availability falls, many body processes slow down.

Thyroid Gland and Normal Thyroid Physiology

The thyroid is a butterfly-shaped endocrine gland located in the anterior neck, below and around the larynx and upper trachea. Its major hormones are T4 and T3. T4 is the major hormone released by the thyroid, while T3 is the more biologically active form at the tissue level.
Thyroid function is controlled through the hypothalamic-pituitary-thyroid axis. The hypothalamus releases thyrotropin-releasing hormone (TRH), which stimulates the anterior pituitary to release thyroid-stimulating hormone (TSH). TSH stimulates the thyroid gland to synthesize and release thyroid hormones.
When adequate thyroid hormone is present in the circulation, it suppresses excessive TRH and TSH secretion through negative feedback.

How Hypothyroidism Develops

Thyroid hormone acts as a major regulator of metabolic activity. When thyroid hormone levels fall, metabolic processes throughout the body become slower. Reduced metabolic activity decreases heat production, contributing to cold intolerance. Reduced energy metabolism contributes to fatigue and weakness.
Reduced gastrointestinal motility can cause constipation, while reduced cardiac stimulation may cause bradycardia and reduced cardiac output. Reduced nervous-system activity may contribute to slowed thinking, poor concentration, lethargy, and excessive sleepiness.

Classification of Hypothyroidism
Primary Hypothyroidism
Primary hypothyroidism results from a problem within the thyroid gland itself. The thyroid cannot produce adequate thyroid hormone despite appropriate or increased stimulation from TSH.
The typical laboratory pattern is increased TSH with decreased free T4. Common causes include Hashimoto thyroiditis, iodine deficiency or excessive iodine exposure in susceptible individuals, previous thyroid surgery, radioactive iodine treatment, certain medications, thyroiditis followed by permanent thyroid failure, and congenital thyroid abnormalities.
Secondary or Central Hypothyroidism
Central hypothyroidism occurs because the pituitary gland or hypothalamic system fails to provide adequate stimulation to the thyroid. The thyroid itself may be capable of producing hormones, but it does not receive appropriate stimulation.
A typical pattern is low free T4 with low, normal, or inappropriately normal TSH. This is why TSH cannot always be interpreted alone, particularly when central hypothyroidism is suspected.
Overt Hypothyroidism
Overt hypothyroidism means there is biochemical evidence of thyroid hormone deficiency. In primary disease, this generally means elevated TSH with low free T4. Patients are more likely to have recognizable clinical manifestations.
Subclinical Hypothyroidism
Subclinical hypothyroidism is characterized by elevated TSH with normal free T4. The patient may have no symptoms or only mild, nonspecific symptoms. Treatment is individualized according to factors such as the degree of TSH elevation, age, symptoms, thyroid antibodies, pregnancy status, and cardiovascular considerations.
Congenital Hypothyroidism
Congenital hypothyroidism is present at birth. It may result from an absent or underdeveloped thyroid gland, abnormal thyroid development, abnormal thyroid hormone synthesis, or certain maternal or fetal factors. Early identification is particularly important because thyroid hormone is essential for normal brain development.

Major Causes and Risk Factors
Hashimoto thyroiditis is an important cause of primary hypothyroidism. It is an autoimmune disorder in which the immune system attacks thyroid tissue. Progressive autoimmune injury can reduce thyroid hormone production.
Iodine is required for thyroid hormone synthesis. Severe iodine deficiency can impair production of T4 and T3 and may cause thyroid enlargement or goiter. Both inadequate and excessive iodine exposure can interfere with normal thyroid function in susceptible individuals.
Thyroid surgery and radioactive iodine therapy can reduce functioning thyroid tissue and subsequently lead to hypothyroidism. Several medications, including amiodarone and lithium, can also interfere with thyroid function.

Clinical Manifestations

Hypothyroidism can produce a wide range of manifestations because thyroid hormones influence multiple organ systems. The overall picture is often one of slowing of physical and mental processes. A patient does not need to have every classical feature.
General Manifestations
- Fatigue and lethargy.
- Weakness.
- Increased sleepiness.
- Reduced exercise tolerance.
- Cold intolerance.
- Weight gain or difficulty losing weight.
- Reduced physical activity.
Skin, Hair, and Facial Changes
Reduced metabolic activity and changes in skin physiology may produce dry, coarse, cool skin, reduced sweating, brittle hair, hair thinning, and hair loss. Some patients develop thinning of the outer eyebrows. A patient with significant hypothyroidism may also develop a puffy face and periorbital puffiness.
The characteristic tissue swelling associated with severe hypothyroidism is called myxedema and reflects mucopolysaccharide accumulation in tissues. It is classically associated with non-pitting tissue swelling.
Cardiovascular Manifestations
Thyroid hormones influence cardiac rate, contractility, and vascular function. Hypothyroidism can therefore produce bradycardia, reduced cardiac output, exercise intolerance, increased peripheral vascular resistance, and possible diastolic hypertension. Long-standing untreated hypothyroidism can also contribute to an unfavorable cardiovascular risk profile, particularly through effects on lipids.
Gastrointestinal Manifestations
Constipation is one of the classic manifestations of hypothyroidism. Reduced thyroid hormone activity can decrease gastrointestinal motility. Patients may also report abdominal discomfort, bloating, or slower digestion.
Neurological and Psychological Manifestations
Thyroid hormone deficiency can affect brain function and may cause slowed thinking, poor concentration, memory problems, slowed speech, excessive sleepiness, depression-like symptoms, reduced motivation, and diminished reflexes. The patient may appear slowed rather than simply tired.

- Muscle weakness.

Reproductive Manifestations
Thyroid dysfunction can interfere with reproductive physiology. Women may experience heavy menstrual bleeding, irregular menstrual cycles, other menstrual disturbances, or fertility problems. Thyroid function is particularly important during pregnancy because maternal thyroid hormone requirements and fetal thyroid physiology change during pregnancy.
Assessment of a Patient With Suspected Hypothyroidism

Nursing assessment should combine history, physical examination, and laboratory findings. Because many symptoms are nonspecific, a systematic assessment is essential.
Important History Questions
- Ask about fatigue, onset, severity, and progression.
- Assess cold intolerance and changes in temperature tolerance.
- Ask about weight changes and appetite.
- Assess constipation and bowel-pattern changes.
- Ask about menstrual changes and reproductive concerns.
- Assess sleep, mood, concentration, and memory.

- Ask about previous thyroid disease, thyroid surgery, radioactive iodine therapy, or neck radiation.
- Review all prescription medicines, over-the-counter medicines, and supplements.
- Ask about personal or family history of thyroid or autoimmune disease.
- Assess pregnancy status or pregnancy plans when clinically relevant.
Physical Examination
Observe the patient's general appearance for lethargy, slow movements, puffy facial features, dry skin, and coarse hair. Measure vital signs and pay particular attention to pulse rate, blood pressure, temperature, and respiratory status.
Inspect and palpate the neck when appropriate to assess the thyroid for enlargement, nodules, tenderness, or asymmetry. A goiter may be present depending on the underlying cause.
Neurological assessment should include mental status, speech, muscle strength, and reflexes when clinically indicated. Delayed relaxation of deep tendon reflexes is a classic examination finding.
Diagnostic Evaluation

TSH
TSH is generally the initial laboratory test used to evaluate thyroid function in suspected primary thyroid dysfunction. In primary hypothyroidism, TSH is usually elevated because the pituitary increases stimulation of the thyroid when circulating thyroid hormone is inadequate.
Free T4
Free T4 measures circulating thyroid hormone that is not protein-bound and is available to enter tissues. The combination of TSH and free T4 is particularly useful for distinguishing primary, subclinical, and central patterns.

T3
T3 measurement is usually not required to diagnose hypothyroidism. A patient can have significant hypothyroidism while the T3 level remains within the reference range. T3 testing is more useful in selected situations, particularly in the evaluation of suspected hyperthyroidism.
Anti-TPO Antibodies
Anti-thyroid peroxidase antibodies may support the diagnosis of Hashimoto thyroiditis. However, antibody positivity alone does not prove that the patient currently has hypothyroidism. A patient can have elevated thyroid antibodies while thyroid hormone levels remain normal.
Other Investigations
- Lipid profile may be considered because hypothyroidism can affect lipid metabolism.
- Electrolytes and glucose may be important when severe hypothyroidism or myxedema coma is suspected.
- Complete blood count and other tests may be ordered when evaluating associated symptoms or alternative diagnoses.
- Additional pituitary evaluation may be required when central hypothyroidism is suspected.
Role of Imaging
Routine thyroid imaging is not required simply to diagnose ordinary hypothyroidism. Ultrasound may be appropriate when there is thyroid enlargement, a palpable nodule, structural abnormality, or suspicion of another thyroid disorder. The diagnosis of ordinary hypothyroidism is primarily biochemical rather than an imaging diagnosis.

The standard treatment for established hypothyroidism is thyroid hormone replacement, most commonly with levothyroxine, a synthetic form of T4. It replaces the hormone that the thyroid is unable to provide adequately.
The levothyroxine dose is individualized according to factors such as age, body size, severity of hypothyroidism, cause, cardiovascular status, pregnancy status, other medications, and laboratory results. The aim is to restore appropriate thyroid hormone status while avoiding excessive replacement.

Treatment Monitoring
After starting levothyroxine or changing its dose, thyroid function is generally reassessed after several weeks because the body needs time to reach a new steady state. TSH is commonly reassessed approximately 4 to 8 weeks after initiation or dose adjustment in primary hypothyroidism, followed by periodic monitoring once an appropriate dose has been established.

Thyroid hormone is essential for normal growth and brain development. Congenital hypothyroidism requires early detection and appropriate treatment to reduce the risk of developmental impairment.
Patients With Cardiac Disease
Patients with significant cardiovascular disease may require cautious thyroid hormone replacement because excessive thyroid hormone can increase cardiac workload and provoke cardiovascular symptoms.

Myxedema coma is a rare but life-threatening decompensation of severe hypothyroidism. Despite the name, the patient does not necessarily have to be literally unconscious. Severe altered mental status may occur.

Patients may have increased sensitivity to cold. Provide appropriate environmental warmth while avoiding unsafe external heating sources, particularly in patients with impaired sensation or reduced responsiveness.
Nutrition and Health Promotion
